Who we are
Attesta is a trading name of Flatlines001 Ltd, which is the data controller for information collected through this website.
What we collect
- Your organisation name and work email address, which we ask for before showing your CQC search results, before showing your readiness score, and before you download a document template.
- Which of those you asked for: the practice you searched for, your readiness score, or the templates you downloaded.
- Business contact details you give us: your name, work email, role, practice or organisation name and, optionally, a phone number.
- Your answers to the readiness scorecard, and the score, readiness band and recommendation calculated from them.
- If you look up your practice, its CQC location identifier and the public information CQC publishes about it.
The scorecard asks about how your organisation operates. We do not ask for, and you should not provide, any patient information or details about named members of staff. The same applies to any template you complete: they are designed to record roles rather than names.
How we use it
We use this information to respond to your request, to follow up about your assessment and our readiness services, and to improve the scorecard and the template library. The lawful basis is our legitimate interest in responding to business enquiries, together with the permission you give when you submit the form.
Knowing which templates are downloaded most often tells us which documents providers actually need, and shapes what we build next. We send marketing email only if you tick the optional box; you can unsubscribe at any time, and untick it without losing access to anything on this site.
Who we share it with
We use trusted service providers to host this website and to manage enquiries. They act on our instructions. We do not sell your information.
How long we keep it

Your rights
You can ask to see, correct or delete your information, or object to us contacting you, at any time. You can also complain to the Information Commissioner’s Office (ico.org.uk).
